Are you using any cream? Protopic? Elidel?
Avoid friction as much as you can. Please see this:

I suggest Pseudocatalse cream + UVB-NB (very low dose).
Check you oxidative stress and lipid peroxidation. There is an urine test for that.
If possible, avoid gluten and dairy. I know it's too much.
